You will lead an integrated care team that delivers a person-centred care model for our seniors, including persons with dementia. You will take the lead in promoting seniors&#8217; independence and maintaining their cognitive and physical functions. You will also ensure that small-group and personalised activities are implemented in the SCC to enable seniors to live with dignity and fulfilment.<span class=\"br\"><\/span>Your job scope includes <\/p>\n<ul>\n<li>Case Management &#038; Administration<\/li>\n<\/ul>\n<ul>\n<li>Complete the Referral\/Screening process and accept\/reject referrals.<\/li>\n<li>Coordinate and co-conduct Initial Assessments, including documenting information on admission suitability and care planning that fulfil the ministry service requirement<\/li>\n<li>Develop and coordinate the delivery of Discharge plans, including referrals to other providers.<\/li>\n<li>Provide input on process improvement activities<\/li>\n<li>Care Planning<\/li>\n<\/ul>\n<ul>\n<li>Coordinate a team to support, bi-annually review and update Individual Care Plans (ICPs)<\/li>\n<li>Lead the development, communication and review of non-clinical aspects of ICPs<\/li>\n<li>Input into process improvement activities for ICP development and evaluation.<\/li>\n<li>Care Delivery<\/li>\n<\/ul>\n<ul>\n<li>Ensure quality of care from the care support team by adhering to Standard Operating Procedures (SOPs).<\/li>\n<li>Investigate and complete documentation for relevant incidents and escalate complex issues to the Cluster Manager timely<\/li>\n<li>Support the collaboration with external agencies, including requests for new services.<\/li>\n<li>Create a supportive and inclusive environment that promotes active social engagement and participation amongst the senior and care staff<\/li>\n<li>Conduct periodic programme audits and evaluations<\/li>\n<li>Stakeholder Engagement<\/li>\n<\/ul>\n<ul>\n<li>Facilitate and complete the intake, admission and discharge processes with Next of Kins(NOKs) or Caregivers.<\/li>\n<li>Co-conduct post-admission onboarding of Seniors with the clinical team.<\/li>\n<li>Update NOK \/ Caregiver on changes to ICPs biannual review or changes in clients\u2018 condition<\/li>\n<li>Address and escalate complaints and feedback from NOKs, Caregivers or Seniors.<\/li>\n<li>Support delivery of events and engagement with stakeholders.<\/li>\n<li>Support Medical Social Worker with Advance Care Plan (ACP) conversations.<\/li>\n<li>Administration &#038; Operations<\/li>\n<\/ul>\n<ul>\n<li>Be accountable for the centre operating expenses and ensure that utilisation is kept within the centre&#8217;s budget.<\/li>\n<li>Be accountable for the utilisation and occupancy targets.<\/li>\n<li>Evaluation of vendor&#8217;s products or services, including managing trialling of and recommendations for new technology or equipment.<\/li>\n<li>Liaise with the town council, building and facilities management on maintenance of centres.<\/li>\n<li>Cascade crisis response SOPs to staff and report to HQ.<\/li>\n<li>Coordinate transportation requests for seniors.<\/li>\n<li>Review and approve roster and leave requests for the Care Team.<\/li>\n<li>Quality Management<\/li>\n<\/ul>\n<ul>\n<li>Monitor staff compliance with Infection Prevention &#038; Control (IPC) guidelines &#038; standards<\/li>\n<li>Prepare and support Internal and External Audits and implement improvement areas.<\/li>\n<li>Cascade and monitor compliance to non-clinical SOPs.<\/li>\n<li>Evaluate the effectiveness of non-clinical SOPs and provide feedback to the Quality Management team.<\/li>\n<li>People Management<\/li>\n<\/ul>\n<ul>\n<li>Lead the team to build up a credible professional image and positive branding of NTUC Health<\/li>\n<li>Provide feedback to the planning, recruitment and evaluation of Volunteer Management.<\/li>\n<li>Accountable for workforce planning and recruitment of staff.<\/li>\n<li>Monitor and ensure staff training needs are met.<\/li>\n<li>Support communication and monitoring of staff performance and compliance to Company policies.<\/li>\n<\/ul>\n<p><b>You should<\/b><\/p>\n<ul>\n<li>Adopt a collaborative approach towards working with internal and external stakeholders.<\/li>\n<li>Possess change management experience and be an effective communicator.<\/li>\n<li>Be proficient in Google Suite Applications and digital tools (i.e. WhatsApp, YouTube, Zoom).<\/li>\n<li>Have a strong sense of adaptability (i.e. being creative and finding new ways of getting things done).<\/li>\n<li>Have a caring, empathetic and patient personality to establish positive relationships with seniors and caregivers.<\/li>\n<li>Possess the ability to lead people and manage clients from diverse backgrounds and levels.<\/li>\n<li>Be proactive and observant in identifying and attending to the needs of seniors and staff.<\/li>\n<li>Possess a growth mindset and embrace continuous learning and development.<\/li>\n<\/ul>\n<p><b>Qualifications:<\/b><\/p>\n<ul>\n<li>Degree holder or equivalent<\/li>\n<li>Possess a minimum of 5 years of experience in managing and leading teams.<\/li>\n<\/ul>\n<p><b>About Us:<\/b><span class=\"br\"><\/span>NTUC Health Co-operative Limited (NTUC Health) is an NTUC social enterprise that provides a comprehensive and integrated suite of quality and affordable health and eldercare services to meet the growing needs of families and their dependents.
