Will play a crucial role in diagnosing and treating a wide range of medical conditions, promoting health education and preventive measures, and coordinating care with specialists and other healthcare professionals. Additionally, will be responsible for practicing population health management and value-based care, focusing on improving patient outcomes through efficient and effective healthcare delivery through specific care programs and clinical pathways, with an emphasis on delivering patient-centered care and optimizing health outcomes.<span class=\"br\"><\/span>Patient Consultations and Management <\/p>\n<ul>\n<li>Conduct comprehensive telehealth consultations and in-person consultations via thorough patient history and physical examinations with patients experiencing Family Medicine and general medicine complaints<\/li>\n<li>Provide healthcare to all family medicine target population across age and gender<\/li>\n<li>Provide chronic diseases and preventive care consultations and follow up to patients as per clinical guidelines<\/li>\n<li>Utilize video conferencing platforms to assess patient symptoms, review medical history, and perform virtual examinations. Develop personalized treatment plans, prescribe medications, and provide ongoing patient education<\/li>\n<li>Deliver high-quality medical consultations to patients<\/li>\n<li>Utilize telehealth platforms for virtual appointments and conduct in-person assessments as needed.<\/li>\n<li>Develop and implement personalized treatment plans, including medication management, referrals to specialists, and lifestyle modifications<\/li>\n<li>Oversee ongoing care for chronic and acute conditions<\/li>\n<li>Adjust treatment plans as necessary based on patient feedback and clinical outcomes.<\/li>\n<li>Collaborate with other healthcare professionals, including primary care physicians, specialists, and nurses, to ensure seamless patient care<\/li>\n<li>Practice population health management by identifying and addressing health issues at the community level, implementing preventive measures, and promoting health education and awareness<\/li>\n<li>Document findings and treatment plans meticulously in patient records<\/li>\n<\/ul>\n<p> Quality Assurance and Compliance <\/p>\n<ul>\n<li>Communicate effectively with patients and their families, providing clear and concise explanations of diagnoses, treatment options, and follow-up care<\/li>\n<li>Adhere to all relevant medical regulations, ethical guidelines, and quality assurance standards for telehealth practice and in-person practice<\/li>\n<li>Maintain up-to-date knowledge of telehealth regulations and best practices. Participate in quality improvement initiatives and document patient encounters accurately<\/li>\n<li>Participate in quality improvement initiatives, research activities, and clinical audits to enhance patient care and outcomes<\/li>\n<li>Regularly review compliance protocols and participate in training sessions.<\/li>\n<li>Collaborate with compliance officers to ensure all practices meet regulatory standards.<\/li>\n<li>Prepare reports for audits and inspections, ensuring accuracy and completeness.<\/li>\n<li>Stay updated on changes in healthcare regulations and implement necessary adjustments<\/li>\n<\/ul>\n<p> Professional Development <\/p>\n<ul>\n<li>Continuously enhance professional knowledge and skills in family medicine, in-person and telehealth best practices<\/li>\n<li>Attend relevant conferences, workshops, and online courses. Stay abreast of advancements in family medicine, in-person care and telehealth technology<\/li>\n<li>Stay up-to-date on the latest advancements in family medicine and telehealth technology<\/li>\n<\/ul>\n<p> Telehealth Platform Proficiency <\/p>\n<ul>\n<li>Maintain accurate and complete patient records in accordance with HIPAA regulations<\/li>\n<li>Maintain proficiency in utilizing the designated telehealth platform, including video conferencing, secure messaging, and electronic health records (EHR) integration Actively participate in training programs and stay updated on platform enhancements and best practices.
