Advantage Hospice is seeking a dedicated and empathetic Hospice Social Worker to join our interdisciplinary team and provide vital emotional, spiritual, and psychosocial support.<span class=\"br\"><\/span><b>Responsibilities:<\/b><span class=\"br\"><\/span><b>Job Purpose:<\/b><span class=\"br\"><\/span>To deliver compassionate, holistic social work services to hospice patients and their families\u2014providing emotional guidance, spiritual support, crisis counseling, and advocacy, while connecting them to essential community resources.<span class=\"br\"><\/span><b>Key Responsibilities:<\/b><\/p>\n<ul>\n<li>Perform initial and ongoing psychosocial, spiritual, and emotional assessments.<\/li>\n<li>Collaborate in developing patient-centered care plans.<\/li>\n<li>Provide individual and family counseling, including crisis intervention.<\/li>\n<li>Serve as a bereavement team member, offering grief support as needed.<\/li>\n<li>Act as an advocate for patients and families in IDG meetings.<\/li>\n<li>Maintain accurate and timely documentation of all services and contacts.<\/li>\n<li>Develop community relationships to enhance referrals and outreach.<\/li>\n<li>Participate in staff meetings and continued professional development.<\/li>\n<\/ul>\n<p> Qualifications:<span class=\"br\"><\/span><b>Qualifications:<\/b><span class=\"br\"><\/span>You may qualify for this role with one of the following: <\/p>\n<ul>\n<li>MSW from a CSWE-accredited school and one year of health care experience.<\/li>\n<li>BSW from a CSWE-accredited school + one year of experience in a health care setting (supervised by an MSW unless exempt).<\/li>\n<li>Bachelor\u2019s in psychology, sociology, or related field + one year of health care social work experience (with MSW supervision).<\/li>\n<\/ul>\n<p> Skills &#038; Traits We Value:<\/p>\n<ul>\n<li>Empathy, integrity, and strong communication skills.<\/li>\n<li>Ability to assess and respond to emotional, spiritual, and psychosocial needs.<\/li>\n<li>Collaborative mindset and a heart for service.<\/li>\n<\/ul>\n<p><b>Physical Requirements:<\/b><\/p>\n<ul>\n<li>Ability to sit, stand, bend, and lift light loads regularly.<\/li>\n<li>Capable of performing duties during extended periods of physical activity.<\/li>\n<\/ul>\n<p><b>Additional Information:<\/b><span class=\"br\"><\/span>This role is integral to the hospice care model and requires flexibility, compassion, and commitment.
